MEDICAL RECORDS
You have the right, under the Data Protection Act 1998 and Access to Health Records Act 1991, to examine your medical records written after November 1991. Your doctor can, however, without informing you, withdraw material that he or she believes would cause serious harm to your physical or mental health. You can ask your doctor or receptionist to see your notes. You may be asked to put your request in writing after which the records have to be made available within 21 days. In certain circumstances there may be a charge (currently up to £10.00) for accessing your records – check with your doctor or doctors’ receptionist about this and any charges that may be made.
You also have the right to see your child’s medical records (up to age 16).

COMPLAINTS AGAINST HEALTH SERVICES
If you have any concerns or complaints about the Health Services you should contact the Health Trust concerned and ask for a copy of their complaints procedure. They may also be able to put you into contact with someone who deals with complaints. You can also contact the Mencap Centre for advice or your local Citizen Advice Bureau.
